Aging Services -Home and Community Based Services - Grand Forks, ND - Basic Care Case Manager I The ND Department of Health & Human Services (HHS) Aging Services Section is seeking a Basic Care Case Manager to assist individuals who reside in or entering Basic Care facilities. This position is a telecommute position and will be assisting individuals served in the: Grand Forks area covering the counties in the northeast portion of the state. As a Case Manager your essential functions of this position will include assessing individuals under the Home and Community Based Service Program who residing in or entering basic care facilities. These duties include assessing functional criteria, assessing individuals who live in the community who have applied to enter a basic care facility, complete care plan and authorizations on an annual basis for individuals residing in basic care facilities. Respond to health, welfare, and safety of individuals in facilities. Provide information to all individuals, power of attorney or guardian, and the basic care facility about overall HCBS and all community services. Assist facilities with transitions into higher level of care such as a nursing home, or into a home environment. Work collaboratively with other sections in the Adult & Aging Services Section, including ADRL transitions, Ombudsman, ADRL intake, Money Follows the Person, Community Service Coordinators, and HCBS Case Managers. Work collaboratively with the LTC MA division regarding MA eligibility. Other duties as assigned by supervisor. To thrive in this position, applicant must be proficient in use of a computer including electronic health records, electronic filing, Microsoft Word, Outlook, and Excel. This position requires great time management, organizations skills, and ability to prioritize tasks. This position requires frequent same day travel. Overnight travel is rare, but may occur for training, etc. This position requires the ability to visit individuals in their own homes or in facility settings and is required to be able to conduct on-site assessments with individuals. To be considered for this position, you are required to have a Bachelor's degree in social work, occupational or physical therapy, gerontology, psychology, vocational rehabilitation, sociology, or certificate in practical nursing; and two year of professional human services work experience in social work, community mental health, public health, behavioral sciences, vocational rehabilitation counselor, addiction counselor, public administration, healthcare management, licensed practical nurse, registered nurse, or closely related in the public or private sector.Location:This position is a telecommute position and will be assisting individuals served in the: Grand Forks area covering the counties in the northeast portion of the state (Grand Forks, Walsh, Nelson, Steele, Traill).
