Life Skills and Transition Center - Grafton, ND - Social WorkerAs a Social Worker, you will provide support to people receiving services at the Life Skills & Transition Center, including those in the Youth Transition Services. Services include psychosocial assessments, individual, group, and family therapy, admission and discharge planning, and consultation. Agassiz Valley Human Service Zone supports the elderly and/or disabled community that wants to continue to be successful in their own home environment. Join our team as a Direct Care Associate!
